Graduate Certificate in Community Hospital Practice (GCCHP)

Graduate Certificate of Community Hospital Practice (GCCHP) is a programme organised by the College of Family Physicians Singapore (CFPS), supported by the Ministry of Health (MOH).

There is currently no intake for GCCHP. The requirements & details will be updated when a new cohort is planned. The details below are archived for your reference only.

Introduction

GCCHP is a 24-months part-time structured training programme designed to train family physicians to be competent in providing care in community hospital setting.

Commencement of the programme is in July each year and training will end in June at the second year of the training. Each trainee will be under the direct supervision of 3 supervisors..

Eligibilities

  • Present trainees currently under GDFM programme or GDFM certificate holders or doctors registered on the Family Physicians Register
  • Full or Conditional registration with the Singapore Medical Council (SMC). Provisional registration doctors are not eligible to apply.
  • Must hold a current and valid practising license issued by Singapore Medical Council (SMC)
  • Must fulfill CME requirements

Course Overview

  1. 80 hours of clinical attachment in participating community hospitals
  2. Family Practice Skills Course (FPSC) on managing patients with complex care needs
  3. Formative assessments to help trainees achieve key competencies in community hospital practice
  4. Summative assessment to validate training

80 Hours Clinical Sessions

  • 20 clinical sessions (4 hours each)
  • 10 sessions per year
  • Trainees and trainers have the flexibility to arrange the clinical sessions themselves
  • Case studies for comprehensive coverage of important areas of care, including:
  1. Rehabilitation Medicine
  2. Subacute Care
  3. Complex & Integrated Care
  • Practice log of 20 cases co-managed by trainees

FPSC in Complex Care

  • Conducted on Saturday & Sunday afternoons
  • Key topics in Community Hospital Practice:
  1. Rehabilitation Medicine
  2. Subacute Care
  3. Complex & Integrated Care
  • Workshops in case management
  • 30 MCQs assessment

Formative Assessment

  • Conducted by trainers after every 10 clinical sessions using EPA (Entrustable Professional Activity).
  • Assessment of Key Competencies in:
  1. Conducting MDM
  2. Discharge Planning
  3. Formulating Care Plans
  4. Family Conference & Communication
  • Conducted by accredited faculty practising in community hospital setting

Summative Assessment

Summative Assessment Eligibilities:

  • Completion of training requirements of GCCHP
  • GDFM or higher qualification in Family Medicine

Summative Assessment Details:

  • Format: Clinical Long Case Exam
  • Conducted in participating community hospitals
  • Typical cases admitted to community hospitals
  • Standard set at EPA level 4 (ready for unsupervised practice with oversight)
  • Assessed by 5 examiners at a community hospital setting
  • Successful candidates will be awarded Graduate Certificate in Community Hospital Practice (GCCHP)
  • Recognized by major community hospitals in Singapore

Graduation Requirements

To graduate, the trainee has to complete all requirements stated in the hand book, and successfully pass a Summative Assessment.
